Job Description
• Manages the care management team serving long-term care members
• Develops, implements, oversees long-term care management policies and procedures
• Assigns, monitors and evaluates work assignments and caseloads
• Manages escalated and complex care cases
• Provides coaching and guidance to long-term care management team
Job Requirements
- Requires a Bachelor's degree
- 5+ years of related experience
- Or equivalent experience acquired through accomplishments of applicable knowledge, duties, scope and skill reflective of the level of this position
