Chronic Care Staffing is Hiring a Remote CCM/RPM Enrollment Coordinator (CMA/RMA)!

Location: Remote (Work from Anywhere in the U.S.)
Pay Range: $17.00 – $20.00 per hour
Employment Type: Full-Time

Overview

Chronic Care Staffing is seeking a detail-oriented and patient-focused Remote CCM/RPM Enrollment Coordinator to support our chronic care management programs. This role is ideal for a Certified Medical Assistant (CMA/RMA) who enjoys engaging patients, explaining healthcare programs, and helping individuals actively participate in managing their chronic conditions.

In this position, you will introduce patients to Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) services, obtain verbal consent, and serve as a trusted liaison between patients and care teams.

Key Responsibilities

  • Conduct outreach to patients for CCM and RPM enrollment
  • Assess eligibility and explain program benefits clearly and professionally
  • Obtain and document verbal consent in compliance with CMS guidelines
  • Educate patients and families on chronic conditions, medications, and care expectations
  • Communicate effectively with providers and internal care teams
  • Promote positive patient experiences through proactive and compassionate outreach
  • Support care plan adherence and encourage self-management strategies
  • Ensure all documentation meets CMS, HIPAA, and CCS compliance standards

Job Requirements

  • Must-Have Qualifications
  • Active, nationally recognized CMA or RMA certification
  • Current BLS certification
  • Direct experience with CCM and/or RPM programs
  • Strong organizational skills and sound professional judgment
  • Excellent verbal and written communication abilities
  • Strong critical thinking and problem-solving skills
  • Proficiency in Google Suite and EMR systems
  • Ability to work independently in a HIPAA-compliant home office environment
  • Nice-to-Have Qualifications
  • Knowledge of CCM regulations, documentation, and billing requirements
  • Experience in care coordination, chronic care management, or transitional care
  • Background in patient and family education for chronic conditions
  • Familiarity with quality measures, reporting, or CMS compliance
  • Additional coursework or certifications related to care or case management
  • Team-oriented mindset suited to a remote work environment
  • Bilingual (English/Spanish) is a plus
