• Onboard new Practices and operationalize a Care plan for their patients • Manage a team of case managers – Delegate & monitor RPM and CCM-related targets assigned to each case manager • Coach and mentor case managers to improve productivity and care being provided to patients • Act as the key point of contact between the Neolytix team and the Practice Provider • Identify patients for specific case management and/or chronic condition management activities using established screening criteria • Apply nursing processes to actively manage members and achieve integrated health services through coordinated service efforts
