About This Role

Nurse Practitioner exists to care for patients as whole people—not just symptoms—by delivering thoughtful, accessible primary care in moments that matter most, both in person and virtually.

In this role, you will:

• Serve patients and families across Arizona, along with internal clinical and care teams
• Be responsible for delivering evidence-based primary care, coordinating treatment plans, and maintaining accurate clinical documentation
• Directly impact patient health outcomes and access to affordable, reliable healthcare

What You’ll Do

In this role, you will:

• Own: Providing in-office and virtual family practice visits, including evaluation, diagnosis, treatment, and follow-up care
• Support: Patients by managing acute and chronic conditions and educating them on preventive care and wellness
• Collaborate with: Medical Assistants, a Medical Director, and an interdisciplinary care team to ensure strong patient outcomes
• Improve: Access to primary care through a mix of in-person and telehealth services
• Advocate for: Patients by delivering thoughtful, patient-first care that removes barriers to treatment

What We’re Looking For

We’re looking for someone who:

• Active Arizona Nurse Practitioner (NP) license, required
• • Multi-State Licensure a plus
• Must be located in Arizona with flexibility to work both in-office and virtually
• Family Practice or Primary Care experience, preferred
• Prior telehealth experience, a plus
• Strong clinical judgment, communication skills, and a patient-first mindset
• Tech-savvy and comfortable navigating EHR and telehealth platforms
• Bilingual (English/Spanish) preferred.
