The Enrollment Specialist's primary purpose is to generate high-quality patient enrollments by making outbound calls to prospective patients to enroll them in TimeDoc's CCM, RPM, BHI, and APCM services. The role involves conducting monthly Chronic Care Management (CCM) phone calls, making 30-50 outbound calls daily, and educating patients on chronic conditions and care plans. Responsibilities also include coordinating care with external parties, identifying gaps in care, updating personalized care plans in the EMR, and accurately documenting all interactions while maintaining HIPAA compliance.
