The Care Coordinator will manage an assigned panel of 400-450 patients, conducting 50-60 daily outbound calls for care plan support and health coaching, while collaborating with clinical teams to address patient needs. Responsibilities also include advocating for patient needs, investigating concerns, and ensuring accurate, real-time documentation while maintaining strict HIPAA compliance.

The coordinator will manage a panel of 300-350 patients, conducting 50-60+ daily phone calls to provide nutritional care plan support and health coaching tailored to specific conditions. Responsibilities also include developing personalized nutritional care plans and collaborating with clinical teams to address patient needs.

The coordinator will educate eligible patients over the phone about insurance-sponsored programs for chronic condition management, such as CCM, BHI, and PCM, while meeting daily call and enrollment goals. They must also advocate for patient needs, address concerns using active listening, and accurately document all interactions in the electronic record system.
