The coordinator meets with care center leadership to assess and develop individualized patient bereavement care plans based on community, patient, and family needs, including conducting post-death risk assessments. This role directs bereavement care plans, provides support and outreach to the grieving community, and collaborates on reviewing program metrics and outcomes with leadership.
