Job Summary
Key Responsibilities:
• Serve as the first point of contact for Hope PACE.
• Triage phone calls.
• Perform administrative duties.
• Perform computer entry to include patient care and demographic data, timesheets, careplans and reports.
• Problem solve in a systematic, organized manner for effective use of the PACE intake process and achieve positive internal and external outcomes.
• IDT responsibilities to include: participation in IDT Team meetings; communication of participant changes to team members; participation in the development of the plan of care; and assistance in the coordination for 24 hour care delivery.
Educational/Training Requirements
High School Diploma
Certifications/Licenses/Registration
Support the assigned team in all daily functions.
Triage phone calls. Phone calls are consistently triaged to correct person. Decisions for triaging are made appropriately. Messages/paging is used effectively. Answer phone within five rings. Phone etiquette is observed at all times. Seek assistance as needed.
Perform administrative duties. Demonstrate proper user of computer, fax, copier and pager and facilitates distribution of faxes. Utilize all office equipment appropriately. Perform clerical duties for team. Filing is complete. Order office supplies and forms as needed and maintain stock. Maintain a neat and uncluttered work space. Maintain client schedules and anticipate future needs.
Perform time management and organizational skills. Prioritize duties on an on-going basis to meet the required standards. Independently prioritize multiple tasks with positive results. Assigned duties are completed within the work day.
Perform computer entry to include patient care and demographic data, timesheets, careplans and reports. Computer input is independently updated as needed and accurately entered in a timely manner. Reports, lists and other data are generated accurately and timely. Distribute all changes of information. Proactively and independently prepare reports. Anticipate needs based on activities of team.
Communicate and collaborate with members of the IDT in care and services to patients/families. Demonstrate collaborative communication.
Coordinate and confirm participant appointments with outside providers.
Provide external/internal customer service.
Record IDT minutes and update agenda on daily basis. Track dates for periodic assessments. Update schedule of assessments when significant changes occur.
Ensure collaboration with HIM for delivery of paperwork to outside providers.
Receive referrals for PACE services and verifies appropriateness of referral.
Maintain required referral and admission data entry and statistics.
Demonstrate ability to review, interpret, and collate clinical information on referrals.
Problem solve in a systematic, organized manner for effective use of the PACE intake process and achieve positive internal and external outcomes.
Utilize IT systems to access and input data intake information.
Conduct information visits with referred potential enrollees.
Participate in the quality process.
Participate in interdisciplinary team, management team, and other related meetings and activities.
Participate in marketing activities.
Maintain required statistical data.

Compensation Pay Range:
$19.67 - $28.34This position requires consent to drug and/or alcohol testing after a conditional offer of employment is made, as well as on-going compliance with the Drug-Free Workplace Policy.
