Role Description

We are looking for an RN to deliver patient centered health coaching and chronic disease management via chat, phone, and video. This role includes:

  • Triage symptoms
  • Provide education on diabetes, hypertension, hyperlipidemia, and weight management
  • Coordinate care with providers and cross functional teams
  • Document patient care accurately in a telehealth setting

Our Registered Nurse Clinical Care Specialists provide direct care and education to patients with chronic cardiometabolic conditions, working closely with our Providers.

Qualifications

  • Strong empathy, communication, and patient-centered care skills
  • Bachelor of Science in Nursing (BSN) required
  • Active Registered Nurse license in good standing, with ability to obtain additional licensure if needed
  • Certified Diabetes Care and Education Specialist (CDCES) or willing to pursue if not currently certified
  • 3+ years of direct patient care in an ambulatory or telehealth setting
  • Adaptable, self directed, and comfortable working independently
  • Proficiency with digital tools for care delivery
  • Flexibility to participate in rotational weekend or holiday coverage
  • A HIPAA compliant, distraction free workspace

Requirements

This role offers a salary of up to $100,000, depending on experience.
