Job Description
Role Description
This position typically works Monday – Friday 8:00am until 5:00pm, 1 weekend a month and 1 holiday a year! This position will allow you to work autonomously and have a more predictable schedule.
The Liaison has three primary job responsibilities:
- Provides clinical assessment of patients referred by the member hospitals for home infusion therapy to determine their capabilities to safely receive services at home with NELC.
- Responsible for developing and implementing strategies to capture all referrals from the member hospital’s system of care (e.g. inpatient, ambulatory, etc.).
- Responsible for relationship building with hospital staff for quality coordination of care between hospital and home.

Qualifications
- Associate degree or diploma in Nursing
- Two (2) years intravenous therapy experience in hospital or homecare
- Excellent patient assessment skills
- RN or LPN license in good standing in the state(s) where providing care/services
- Valid driver’s license
- Excellent driving record
Requirements
- BS in Nursing preferred
- Current BLS Certification preferred
- Knowledge of HIPAA Privacy and Security Regulations preferred
- Knowledge of JCAHO accreditation process and overall JCAHO standards preferred
- Working/practical knowledge of home infusion therapy administration/equipment use preferred
