- Connect with patients and leverage your case management skills
- Make a significant impact on their recovery journey
- Assess medical needs and coordinate care through comprehensive plans
- Participate in facilitating patient progress
- Collaborate with healthcare providers and employers to ensure optimal outcomes
- Document patient interactions and monitor treatment plans
- Ensure compliance with Workers' Compensation regulations
Qualifications
- Strong clinical nursing skills, particularly in Case Management and Occupational Health settings
- Excellent critical thinking and problem-solving abilities
- Experience with evidence-based guidelines and ODG standards
- Effective communication skills
- Organizational skills for managing multiple cases and maintaining accurate documentation
- A commitment to teamwork
- Adaptability and a forward-thinking mindset
