Job Description

• Manage inbound referrals from various sources and promptly initiate the admissions and intake process. • Conduct detailed intake conversations in both English and Spanish with individuals in complex situations, including those recently discharged from hospitals, experiencing homelessness, or involved in the justice system. • Ask structured screening questions to assess client needs and determine appropriate next steps. • Obtain and process necessary documentation, including Releases of Information (ROI), ensuring all paperwork is accurate and complete. • Coordinate evaluations, prescriptions, and follow-up services with providers and peer support teams. • Maintain consistent communication via phone and email to guide clients through each stage of the referral and admissions journey. • Handle sensitive and high-emotion conversations with empathy, professionalism, and composure. • Document all client interactions and case updates within CRM systems such as Salesforce. • Follow up persistently to ensure referrals are successfully converted into active cases.

Job Requirements

  • Bilingual (English/Spanish): Fluent and comfortable conducting detailed conversations with diverse clients.
  • Empathetic & Professional: Patient and compassionate when supporting individuals facing mental health challenges, housing instability, recent incarceration, or crisis situations.
  • Strong Intake & Coordination Skills: Able to gather accurate information, manage documentation (including ROIs), and coordinate evaluations and provider communication efficiently.
  • Resilient Under Pressure: Calm, solutions-focused, and professional in emotionally intense or complex situations.
  • Organized & Detail-Oriented: Thorough with paperwork, follow-ups, and case tracking.
  • Tech-Savvy: Comfortable using CRMs such as Salesforce to document interactions and manage workflows.
  • Experience: General work experience is required. Experience speaking with Americans in a professional, polite manner is a plus, but not necessary.
