• Serve as the primary point of contact for patients enrolled in the Collaborative Care program. • Support the mental and physical health of patients on an assigned caseload, coordinating closely with the patient’s medical provider and, when appropriate, other mental health professionals. • Participate in regular caseload consultations with the Psychiatric Consultant and communicate resulting treatment recommendations to the patient’s medical provider. • Screen and assess patients for common mental health and substance use disorders using standardized tools, and facilitate patient engagement and follow-up care. • Provide education to patients on common mental health and substance use disorders, including available treatment options. • Deliver brief, evidence-based behavioral interventions such as behavioral activation, problem-solving treatment, motivational interviewing, and other appropriate therapies. • Track treatment response, monitor changes in clinical symptoms, and identify any treatment side effects or complications. • Support psychotropic medication management prescribed by medical providers, focusing on adherence, effectiveness, and side-effect monitoring. • Track patient follow-up and monitor clinical outcomes. • Document patient progress and treatment recommendations in the EHR and other required systems, ensuring information is shared with medical providers, the Psychiatric Consultant, and other treating professionals. • Adjust treatment plans for patients who are not improving as expected in consultation with the medical provider and Psychiatric Consultant, including referral to more intensive or specialized care when needed. • Facilitate referrals to evidence-based psychosocial treatments and other clinically indicated services outside the organization. • Develop relapse prevention and self-management plans for patients who have achieved their treatment goals and are preparing to be discharged from the caseload.
