Role Description

Banyan Treatment Centers is seeking a Telehealth Care Specialist to support clients throughout their treatment journey. This role assists with client intakes, care coordination, communication with admissions teams, and collaborating with therapists to help ensure clients receive the support and resources they need. The Telehealth Care Coordinator plays a role in maintaining clinical operations, supporting patient engagement, and ensuring timely, compliant documentation while collaborating closely with the multidisciplinary treatment team.

Position Details

  • Reporting To: Manager of Telehealth Operations
  • Schedule: Full-time (Monday & Wednesday, 8 AM-4 PM and Tuesday, Thursday & Friday, 10:30 AM-6:30 PM)
  • Location: Remote

Key Responsibilities

  • Telehealth Intake & Care Coordination:
    • Conduct comprehensive telehealth intake assessments, including clinical screenings and information gathering, to determine client needs and appropriate program placement.
    • Coordinate closely with Admissions, Business Development, and Telehealth Clinical teams to ensure a smooth transition from intake through treatment initiation.
    • Apply professional judgment to review client assessments and assist in identifying appropriate services, treatment recommendations, and care pathways.
    • Serve as a key point of contact for internal teams to support communication and coordination of telehealth services.
  • Patient Engagement & Support:
    • Provide clients with education on program expectations, treatment processes, and available services.
    • Conduct scheduled individual check-ins or supportive sessions to monitor progress, address barriers to care, and reinforce treatment goals.
    • Assist therapists with treatment planning, client assignments, and coordination of services within the telehealth program.
    • Promote client engagement, accountability, and retention throughout the treatment experience.
  • Documentation, Compliance & Accreditation:
    • Maintain accurate, timely, and complete documentation related to client intakes, care coordination activities, and treatment updates within required systems.
    • Ensure all documentation complies with organizational policies, regulatory requirements, and confidentiality standards.
    • Communicate updates and client progress with internal teams through systems such as Salesforce and the electronic medical record.
    • Participate in team meetings, case reviews, and program initiatives to support the ongoing development of telehealth services.

Qualifications

  • Associate's degree or higher in Social Work, Psychology, or another related Human Services field.
  • Two to three years of work experience in behavioral health, mental health, and/or substance use disorder treatment settings, preferred.
  • Adherence to the Healthcare Code of Ethics and professional standards of conduct.
  • Knowledge of state and federal confidentiality regulations.
  • Familiarity with Joint Commission standards and documentation expectations.
  • Strong organizational, communication, and clinical support skills.
  • Ability to work effectively as part of a multidisciplinary treatment team.
