Role Description

The Exercise Physiologist is responsible for providing exercise guidance to disease management members.

• Collaborates with nurse case managers in managing disease management health goals of members.
• Implements the primary care physicians' exercise recommendations providing safe and effective exercise instruction and guidance.
• Provides member health coaching, education, assessment, and support of behavioral changes to promote self-management of chronic conditions.
• Provides members with education materials related specifically to member score condition.

Qualifications

• Requires a BS/BA degree in exercise science, exercise physiology, kinesiology, physical therapy, or physical education.
• A minimum of 3 years experience as an exercise specialist, exercise physiologist, or fitness instructor/trainer developing exercise prescriptions for special populations.
• Any combination of education and experience which would provide an equivalent background.

Requirements

• A minimum of 1 year cardiac rehab, pulmonary rehab, and physical therapy experience in a clinical setting preferred.
• Certification by the American College of Sports Medicine (ACSM) as a Health Fitness Instructor preferred.
• MS in a related field and membership in American Society of Exercise Physiologist preferred.
• Telephonic experience is strongly preferred.
