Job Description

• Provide care coordination services to Providence Health Plans (PHP) members • Disease management programs, including educating, motivating and empowering members to manage their disease • Triage and referral, transition of care planning, end of life care planning, and other support • Advocate for and assist the member in the achievement of optimal health and access to care • Manage member health plan benefits and assist finding alternative services when benefits are exhausted

Job Requirements

  • Certificate/Diploma from a state approved practical nursing program Major - Nursing
  • Associate's Degree: Healthcare or healthcare related field
  • Upon hire: Oregon Licensed Practical Nurse
  • 5 years clinical nursing experience
  • 2 years experience working with physicians in the collaboration and management of patient care
