Job Description

• Manage inbound referrals from health system and provider partners, ensuring accurate intake and documentation • Engage referred patients and assist with scheduling their Midi appointments • Ensure referrals move through established workflows efficiently and without delays • Serve as a point of contact for patients navigating care between Midi and its health system and provider partners • Answer patient questions about Midi services, referral pathways, and next steps in care • Provide clear, supportive communication to ensure a positive patient experience • Manage the dedicated Zendesk inbox and voicemail for inbound referral communications • Share updates and operational feedback with the Commercial team regarding referral activity and workflow improvements

Job Requirements

  • 2–4 years of experience in care coordination, referral management, patient navigation, or patient support
  • Experience working in a medical practice, healthcare organization, or digital health environment
  • Daily hands-on experience using Zendesk or a similar ticketing platform to manage patient or customer support queues
  • Strong experience communicating with patients via phone and written channels
  • Comfortable managing high volumes of inbound requests and patient communications while maintaining a positive patient experience
  • Ability to follow structured workflows and SOPs while identifying opportunities to improve operational processes
  • Experience working with EHR systems and operational tools
  • Strong communication and cross-functional collaboration skills
  • Comfort working in a fast-paced, growing healthcare organization.
