The Social Worker III supports individuals with traumatic brain injury (TBI) and/or spinal cord injury (SCI) by collecting information, developing person-centered rehabilitation plans, and coordinating necessary goods and services. Responsibilities also include providing case management, continuous reassessment, discharge planning, and counseling to individuals and their families.

The therapist will provide evening hours counseling and therapy treatment remotely via telemedicine to patients across Arizona, utilizing evidence-based practices like CBT and DBT in individual and group settings. Key duties include maintaining patient clinical records, conducting psychosocial assessments, developing treatment plans, and performing crisis intervention as needed.
